What is tympanoplasty?

Generally, eardrum perforations result from trauma to the eardrum. Tympanoplasty consists of repairing the hole in the eardrum with a graft or prosthesis. Aside from the tympanic membrane, other parts of the ear canal, such as the middle ear bones, can also be injured. During tympanoplasty surgery, the damaged parts of the ear canal are removed and replaced using a graft or prosthesis.


Most commonly, the surgeon removes the perforated eardrum and replaces it with a membranous tissue graft from the back of the ear during a tympanoplasty, depending on the complexity of the surgery.

Tympanoplasty treatment Procedure

Diagnosis

A tympanoplasty diagnosis is determined by a physical examination and hearing tests. During the physical examination, the ENT physician collects your medical history to determine your hearing loss and its causes. In order to get a clearer view of the eardrum, he will insert an otoscope into the ear canal. Once that is done, he will perform diagnostic tests such as:


  • Tuning fork test- Tuning forks are two-pronged metal instruments used to produce sound when struck together. A doctor will use them next to the ear to assess the patient's hearing ability.

  • Audiology- During this test, the doctor will play sounds at different volumes and pitches to determine how severe the hearing loss is.

  • Swab test- If there is any ear discharge, the doctor will collect a sample and culture it in a lab to determine if there is an infection.

  • Tympanometry– A tympanometry test measures the eardrum's response to air pressure changes. The doctor will note the measurements of the tympanum to determine how severe the perforation is.

Procedure

The surgeon will help you prepare for your surgery once you have been diagnosed. Ear surgery can be performed openly or endoscopically. Endoscopic surgery leaves a small, barely noticeable scar behind and in front of the ear.


The surgery is performed as an outpatient procedure, typically with local anaesthesia. If a tissue graft is needed for eardrum repair, it is taken from the back of the ear due to its similarity and proximity to the surgical site. Once you are under anaesthesia, the surgeon will either make an incision behind your ear or work through your ear canal. After accessing your eardrum, they will use a graft made from your own cartilage, connective tissue, or synthetic material to fill the hole. The incision will be closed with bio-absorbable stitches and packing will be placed in the ear canal to secure the graft. Following the surgery, you will be monitored in a recovery area.

Preparation for Surgery

It is important to prepare for a tympanoplasty as it is an invasive procedure. Your healthcare provider should know everything you know about your medical history, including any medications or supplements you take regularly. Please also let him know if you have any allergies.


The sudden changes in your child's hearing can make them feel scared during the surgery and afterwards. For kids who are too young, you can speak with a paediatrician or a paediatric therapist to help you prepare accordingly. Depending on the child's age, you should talk with them to prepare them for the surgery.


On the day of surgery, wear loose, comfortable, and easy-to-remove clothing since you may be required to wear a hospital gown. Remove all jewellery before coming to the clinic and do not wear makeup, deodorant, or nail polish.


Before your surgery, you should stop smoking at least 2-3 weeks before it so that it won't negatively impact your recovery. You should also ask your surgeon about any dietary precautions you should take.
