What are the reasons for treating Bartholin cysts?

In the labia, there are glands called Bartholin glands that release fluid via a duct. This fluid is intended to lubricate the vagina. When the ducts get blocked, the fluid backs up and forms a cyst. It is called a Bartholin cyst, and it appears on the vaginal wall as small, fluid-filled lumps.


Women of reproductive age are more likely to develop bartholin cysts than women after menopause.

There is usually no specific treatment for asymptomatic Bartholin's cysts. The cysts need to be treated promptly if they are larger and recurrent, or else they can cause bacterial infection such as Escherichia coli (E.coli) and other sexually transmitted infections like gonorrhoea and chlamydia. If the cyst is not treated, it may also develop an abscess.

Bartholin Cyst Diagnosis and Treatment

Bartholin cyst diagnosis

The following diagnostic tests may be recommended by a gynaecologist after evaluating your medical history:

  • Pelvic exam- They are usually recommended when you experience unusual vaginal discharge and pelvic pain. The test evaluates the reproductive organs (vagina, cervix, uterus) for any abnormalities.

  • Needle biopsy– A Bartholin cyst is mainly performed to obtain a sample of cells and check for cancerous cells. Postmenopausal women are more likely to develop cancerous Bartholin cysts.

Bartholin cysts treatment is generally categorised into two methods based on the size, pain, and infection of the cyst.


Non-surgical treatment

Medicines- A few antibiotics and painkillers are recommended by doctors to manage pain and fever. These antibiotics include ceftriaxone, ciprofloxacin, doxycycline, and azithromycin.

Sitz bath- A sitz bath involves soaking the vaginal region in warm water for a few minutes. For better and quicker relief, doctors recommend adding an antibiotic solution to warm water. This helps in managing the pain as well as softening the cyst. In order to achieve better results, doctors usually recommend taking sitz baths three to four times a week.


Surgical treatment

In the event that the Bartholin cyst causes some serious complications, surgical intervention is recommended.

Bartholin gland removal- A gynaecologist may recommend surgical removal of the Bartholin gland when it is suspected that it may be cancerous or recurring and quite painful. Surgical removal of the entire Bartholin gland is very rare and is dependent on the severity of the condition. After surgery, there is a risk of bleeding or complications.


Surgical drainage- It is possible to drain a cyst through an incision using surgical drainage. During this procedure, doctors numb the site with local or general anaesthesia and then make a small incision to drain the pus. As soon as the drainage is completed, the drainage tube (rubber tube) is removed.


Marsupialization- The surgery is performed under local/spinal anaesthesia for recurring and extremely painful Bartholin cysts. An incision is used to drain the cyst, and once the cyst is drained, the incision becomes slightly enlarged and appears to be a kangaroo pouch on the vaginal lips, which allows any further fluid to drain out.

Precautions

In order to reduce the risk of infection and enhance quick healing, gynaecologists recommend avoiding the following things after surgery.

  • To avoid infection and further complications, avoid sexual activity until the area is fully healed.

  • Perfumed bath products can irritate the skin and cause infections, which can delay healing.

  • During the first 24-48 hours following surgery, avoid driving and any physical activity that requires careful attention.

  • Wear cotton clothes while your body is healing, as tight clothes can cause discomfort around the surgical site.

  • Swimming and bubble baths should be avoided.

Expectations about surgery

Surgery for Bartholin cysts is done under anaesthesia and takes a short amount of time. Post-surgery, you can expect to experience the following:

  • For the next few days, you will feel pain and discomfort in your vulva.

  • Sitting for long periods of time causes discomfort.

  • If your urine comes into contact with the wound, you will experience a burning sensation.

  •  Tired easily and need painkillers for a week or two.

It may take between two and four weeks for a complete recovery to occur.
