What is Molar Pregnancy?

Diagnosis and Treatment for Molar Pregnancy
Diagnosis- Molar Pregnancy
The diagnosis of a molar pregnancy is ideally made during regular prenatal visits. However, if the patient experiences symptoms indicating a molar pregnancy, the doctor may suggest any of the following tests.
Blood tests- In molar pregnancy, the level of hCG, a pregnancy hormone, increases compared to normal pregnancy.
Transvaginal ultrasound- The test is used to produce a detailed picture of your uterus. It scans the tissue within the uterus and helps determine whether cysts are present.
Urine test- The urine test helps to identify the colour and composition of urine as well as the level of hCG. A high level of hCG in urine indicates molar pregnancy tissue in the body.
Healthcare providers may also check for other medical problems if a molar pregnancy is detected.
Treatments- Molar Pregnancy
Medications and surgery can be utilised to treat molar pregnancy. However, medical termination is not commonly recommended due to the need for continuous monitoring, repeated follow-ups, and a longer treatment period.
In contrast, if the molar pregnancy is detected earlier in the cycle without unstable bleeding, the patient prefers medical management.
Surgery- Molar pregnancy can be removed with a surgical procedure known as D&C, which involves a methotrexate injection. This surgery is carried out while the patient is under general anaesthesia, ensuring a painless experience. During the procedure, medications are administered to dilate the cervix, which typically takes 30-40 minutes. Once the cervix is sufficiently dilated, the surgeon utilizes a curettage instrument to remove all pregnancy tissue from the uterus. The cervix then naturally contracts, leading to termination of the pregnancy without any incisions or stitches necessary.
Medication- An injection of methotrexate medicine is usually used to treat molar pregnancy. It is given directly to the patient in a single dose. After the procedure, doctors monitor the level of hCG. A second dose may be required if the first dose fails to terminate the pregnancy. An ultrasound confirms the termination.
Hysterectomy (uterus removal)- A laparoscopic or open-cut incision can be used to perform the procedure when the female has a high risk of gestational trophoblastic neoplasia (GTN).
Preparation before surgery
To prepare well for the surgical treatment of molar pregnancy, follow these tips:
Ensure a smooth operation by discussing all your current medications, supplements, and herbal potions with your consultant surgeon.
To avoid anaesthesia-related complications, it is recommended not to drink anything for at least 4-6 hours before the surgery.
After surgery, carry some loose cotton clothes to change into.
For post-surgical vaginal bleeding, carry a pack of sanitary pads.
Precautions
To avoid further complications, you should take these precautions for at least two weeks.
Use Sanitary pads for post-surgical vaginal bleeding- The use of sanitary pads prevents leakage of post-surgical vaginal bleeding and is safer than using tampons or menstrual cups.
Avoid using tampons- The use of tampons after molar pregnancy surgery can increase the risk of microbial infection.
Avoid using scented soaps- The harsh chemicals in scented soap can damage your skin and increase the risk of infection.
