Squint surgery or eye muscle surgery: what is it?

The eye surgeon or ophthalmologist loosens, tightens, or relocates the eye muscles to correct structural issues that lead to crossed eyes in a minimally invasive procedure.


The surgery is performed on an outpatient basis and takes around 40 to 60 minutes. When performed on a child under six, strabismus surgery results in the best results. It is also effective in adults, but the corrected eye can deviate significantly.

Complications & Risks

It is generally safe to undergo strabismus surgery, but there are some risks and complications involved. Some common problems include-

  • Anesthesia or infection in the eye can cause immediate side effects that can be addressed without long-term consequences.

  • It is possible for the eye structure to be damaged, or severe endophthalmitis (eye inflammation) to develop, resulting in vision impairment.

  • Exophthalmos, also called bulging eyes, is a condition caused by scarring behind the eye that pushes the eye forward due to muscle relocation.

  • After surgery, eye deviation can be increased as a result of overcorrection or undercorrection of strabismus.

Strabismus surgery can also lead to minor complications such as infection, swelling, or breathing problems. The surgeon takes all potential scenarios into consideration and resolves them properly so that the patient can go home safely.

During surgery, what happens?

In the preoperative area, the patient's temperature, pulse, blood pressure, respiratory rate, and oxygen levels are checked before taking them to the operation theater (OT).

In order to induce sleep or drowsiness, an intravenous (IV) line is attached to the arm or hand and the patient is brought into the operating room (OR).

Following are the steps involved in the surgery:

  • First, a lid speculum is placed in the eye to keep it open. If both eyes need to be repaired, the speculum is placed in both.

  • In order to locate the muscles that need to be repaired, the surgeon will rotate the eye and make an incision in the conjunctiva.

  • The muscles are clearly visible after the incision has been made.

  • In order to correct the abnormality in the eye muscle, techniques such as resection, recession, or plication will be used.

  • During resection, muscle fibers are cut to shorten or make the muscle smaller. Then the muscle is stitched back together.

  • In a recession, the eye muscle is separated from the eye and reattached at a different place.

  • In plication, the eye muscle is folded and stitched to form a new shape and size.

  • The eye muscle is typically secured with adjustable sutures.

Squint surgery benefits

Strabismus surgery has the following benefits:

  • Strabismus surgery improves ocular alignment by allowing the patient to look in the same direction.

  • With squint surgery, the field of vision of both eyes overlaps, allowing depth perception, and a 3-D image of the object is created. Patients with strabismus cannot perceive depth because their field of visions do not overlap.

  • In adults, if diplopia or double vision has occurred, the surgery will address this problem and eliminate strabismus as a cause of double vision. If there is another cause of double vision, the condition will persist after surgery.

  • Binocular Visual Field Expansion- A squint eye often limits the peripheral vision to 120 degrees, preventing the patient from looking at the sides without turning. The surgery resolves this problem and expands the vision to 190 degrees.

  • As a result of crossed eyes, many people with strabismus feel self-conscious and self-conscious. Strabismus surgery also has cosmetic benefits, as the eyes will appear normal. Patients will therefore feel less restrained and will have a better self-image as well.

  • In order to optimize their vision and achieve binocular vision, patients with strabismus often adopt an abnormal head posture (Ocular Torticollis). Following surgery, the patient will no longer have to maintain the abnormal position, and over time the habit will improve.

Untreated strabismus can lead to serious consequences.

Strabismus in children is usually not serious for up to 4 months. The longer the eyes remain crossed, the more the brain ignores images from the abnormal eye. If it persists, it can lead to several problems. The brain will again have a hard time controlling the abnormal eye if the treatment is delayed.

The following problems can occur if squint eye is not treated as soon as possible:

  • The affected eye suffers from lazy eye or amblyopia, which results in permanent poor vision.

  • Dual image formation in the eyes causes double vision or poor 3D vision.

  • Headaches caused by improper eye function.

  • Your regular life will be affected by blurry vision.

  • As the unaffected eye has to work harder for visual perception, it causes eye strain and fatigue.

  • Self-esteem and confidence are low due to the appearance of the eyes.

Strabismus can also be caused by a brain tumor, which may go undetected if not treated right away.
