What Is PCNL?

Percutaneous Nephrolithotomy (PCNL) is a minimally-invasive treatment for large kidney or upper ureter stones that are unresponsive to standard techniques like Shock Wave Lithotripsy or Laser Ureteroscopy. This procedure effectively relieves painful symptoms caused by significantly enlarged stones and has a high success rate in treating stones measuring 2 cm or larger. 


Its minimally-invasive approach results in minimal discomfort for patients and fewer complications, allowing them to quickly resume their daily activities without inconvenience. As a result, PCNL has become the preferred method over traditional open surgery for treating these types of stones.

Diagnosis

The doctors recommend some diagnostic tests before PCNL surgery, as they do with any other surgery. A number of common diagnostic tests are performed before PCNL surgery to determine the size and number of stones and to rule out underlying conditions or diseases.

Procedure

The steps involved in PCNL surgery are as follows:

  • Before surgery, the patient is sedated with general anaesthesia. This keeps the patient asleep throughout the procedure.

  • To determine the exact location of the stone, the surgeon makes a small incision in the flank area and inserts a nephroscope under x-ray guidance.

  • To access the kidney's urine-collecting system, the surgeon uses a thin needle and a guidewire to gain safe access with the nephroscope.

  • As soon as access is gained, the surgeon extracts the stones. Microforceps are sometimes used to remove the stones; however, if the stones are too large, a laser might be used to break them into smaller fragments.

  • Lastly, a drainage catheter is placed after all the stones are removed and contrast is used to confirm there are no stones or fragments left in the ureter.

  • Some surgeons use DJ stents, thin and tiny tubes that ensure that swelling does not affect urine drainage. These tubes are usually kept for 10 to 14 days and then removed.

  • Before surgery, your doctor might ask you to stop taking certain medicines, such as blood thinners.

  • To prevent allergic reactions to anaesthesia, make sure your doctor is aware of your allergy to anaesthesia.
