What Is RIRS Surgery?

RIRS, also known as Retrograde Intrarenal Surgery, is a minimally invasive technique used to extract large or complex kidney stones that cannot be treated with lithotripsy. The procedure involves utilising the urethra as a pathway to reach the kidneys.

Instead of making cuts or holes, surgeons use an optic endoscope and laser to locate and pulverise the stones. This approach has proven to be highly successful with minimal risk of complications.

Diagnosis

In order to assess the size and location of the stones, underlying diseases, and the patient's overall health, surgeons recommend some diagnostic tests before performing RIRS. RIRS patients need to have their test results on the day of surgery. Here are the most common diagnostic tests performed before surgery:

  • Blood test

  • Urinalysis

  • Blood urea nitrogen (BUN) test

  • Imaging tests (X Rays, abdominal ultrasound, MRI)

Procedure

The following steps are involved in RIRS:

In this procedure, anaesthesia is used to keep the patient sedated throughout the surgery. The doctor can use spinal anaesthesia or general anaesthesia, depending on the patient's preference.

To reach the urine-collecting part of the kidneys, the surgeon inserts an endoscope, which is a long and flexible tube, into the urethra. The images are displayed externally on a screen by the doctors during this x-ray-guided procedure. The surgeon can perform the procedure more precisely as a result.

When the surgeons locate the kidney stones, they use a laser probe to break them and reduce them into fine fragments using the endoscope.

Through the same passageway, broken fragments are collected in a 'stone basket' and eventually extracted.

Double J-Stunting in RIRS

Before or after the surgery, surgeons may use double J stents- ureteral stents. The endoscope can pass more easily through the ureter if the stent is placed before the surgery to dilate the ureter. After the surgery, the stent may be placed for up to 10 days to promote healing.

Preparation for Surgery

It is crucial to prepare for RIRS in order to ensure a seamless procedure. Preparation is crucial for RIRS as it contributes to the efficacy and reduces the possibility of complications. In order to prepare for RIRS, your doctor will provide you with some instructions. Here are some tips:

  • Anaesthesia effects can be impacted by eating or drinking anything 8 to 9 hours before surgery.

  • It is recommended that you stop smoking a couple of weeks before the surgery, as it can affect the efficacy of the surgery.

  • It is important that you discuss the treatment procedure with the doctor beforehand so that you are prepared for what the procedure will entail.

  • Don't forget to tell your doctor about any medicines you're taking and if you are allergic to anaesthesia.

Benefits of RIRS Surgery

An advanced treatment technique for kidney stones, RIRS has a high success rate. Here are some of its benefits:

  • There is a very low morbidity rate.

  • There are no cuts or stitches involved in this minimally invasive procedure.

  • Blood loss is minimal and the procedure is painless.

  • The renal tissue is not damaged by this technique.

  • Recovery is quick and hassle-free.

  • Within a couple of days, patients can resume their normal lives.
