What is Uterine Fibroid?

A uterine fibroids is a non-cancerous growth of the uterus that often appears during the childbearing years. They can vary greatly in size, shape, and location. In addition to appearing in the uterus or on its surface, uterine fibroids can also show signs and symptoms. They are generally painless and do not present any symptoms.


Fibroid medication and surgical treatment are available to treat uterine fibroids, depending on the severity of the condition, the number of fibroids, and their size.

Diagnosis

In order to identify the number of fibroids, size of the fibroids, and exact location of the fibroids, the doctor recommends some diagnostic tests. These tests include the following:


Hysteroscopy: A probe is placed through the cervix to examine the uterus cavity in order to detect submucosal leiomyomas.


Hysterosalpingography: An X-ray and dye are used to examine the uterus and fallopian tubes in individuals suffering from infertility.


Abdominal and pelvic CT scan: In this procedure, soft tissues, blood vessels, and internal organs are examined non-invasively.


Blood tests: The doctor may recommend a Complete Blood Count (CBC) and determination of oestrogen/progesterone levels in the blood. A blood test may also identify underlying health conditions that may complicate treatment.


Open biopsy of tumour: A tissue biopsy is sent to a pathology lab for a pathological examination, which examines the biopsy sample under a microscope. After combining clinical and microscope findings, the pathologist makes a definitive diagnosis.


Ultrasound scan of the pelvis: Using high-frequency sound waves, it produces real-time images. Doctors usually recommend it to determine the exact location of fibroids.

Treatment

A woman's treatment options for uterine fibroids are determined by a variety of factors, including the severity of the symptoms, the location of the fibroids, her age, whether she is pregnant, and whether she wishes to have children in the future. For uterine fibroids, there are two main treatment options: medication and surgery.


Medical treatment for uterine fibroids

There are several medications that can shrink fibroids, but they may not necessarily eliminate them. Common medications include:


Gonadotropin-releasing hormone agonists (GnRH): In order to treat fibroids, these drugs temporarily mimic menopause by blocking oestrogen and progesterone synthesis. As a result, the fibroids diminish, the anaemia usually gets better, and the menstruation ceases. In spite of this, these drugs should not be taken more than three to six months at a time since long-term use can cause bone loss.


Nonsteroidal anti-inflammatory drugs (NSAIDs): In spite of the fact that these medications are not hormonal treatments, they may reduce pain quite effectively, but they do not reduce the bleeding caused by fibroids.


Tranexamic acid: The use of drugs in this class, such as Lysteda or Cyklokapron, can help prevent heavy menstruation, but should only be done when significant blood loss is anticipated.

Surgical Treatment

Hysteroscopic myomectomy: This procedure is recommended for the removal of submucous fibroids. In this procedure, fibroids are removed using an instrument called a hysteroscopic resectoscope, which is passed into the uterine cavity through the vagina and cervical canal, and the fibroids are removed using an electrosurgical wire loop.


MRI-guided focused ultrasound surgery (FUS): Your uterus, along with the other organs inside your abdomen, will be clearly visible by the doctor during this procedure, which involves placing you inside an MRI scanner. In order to heat up the fibroids before completely destroying them, your uterus will remain intact after the doctor detects the exact location of the fibroids using a transducer.


Myolysis and Cryomyolysis: By making small incisions, the doctor recommends this procedure for fibroids of a certain size near the uterine surface. An electrical current or laser is used in this procedure to close the blood supply to the fibroids through the use of surgical devices inserted through the incisions. Fibroids are left intact and do not require surgery. However, if they do not receive a blood supply, they gradually shrink.


Endometrial ablation: Fibroids that cause heavy and abnormal bleeding are treated with this procedure. In the procedure, an instrument is inserted into the uterus, which uses heat, microwave energy, hot water, or electric current to destroy the uterine linings. If you are pregnant or plan to become pregnant in the future, endometrial ablation is not recommended.


Radio-frequency ablation: This procedure uses heat to effectively target fibroids and decrease their size by shrinking their blood supply. It is typically recommended for patients with smaller fibroids, a uterus positioned below the navel, and no history of extensive abdominal surgeries. While it can greatly reduce the size of fibroids, complete elimination is not always guaranteed and there is a possibility of regrowth or development of new fibroids afterwards.
